Also, Dual Choice Plans are available for Groups. DINA Dental plans are designed to provide comprehensive coverage with maximum savings without compromising quality of care for both individual and group policies. All plans also offer access to a FREE Vision and Prescription Drug Card Plan.

DINA Dental - PPO Plan Highlights
- Use any dentist or a Dina PPO Dentist
- Dina PPO Dentist are contracted, credentialed, and have agreed to charge pre-determined fees for procedures
- Co-payments may not exceed certain discounted dollar amounts. (Usual, Reasonable and Customary charges may apply when accessing a dentist who is not participating in the network.
- Available to Groups or Individuals
- Only 2 Employees Qualify as a Group
- Immediate Coverage for Type I and Type II Plans
- Discounts on Orthodontics
- Benefit Year Maximum Increases to $1500 in the 3rd Benefit Year
- Qualifies for Section 125 (Cafeteria Plan) deductions
- Operating in Louisiana since 1978
- This plan is an insurance program that is fully regulated by the Lousiana Department of Insurance

Dina Dental - Full Indemnity Plan Highlights
- The Full Indemnity Plan lets you choose your own dentist - in fact, there is no network for Full Indemnity Plan
- Groups as small as 5 full-time employees qualify
- Immediate coverage for preventive procedures, simple extractions, and fillings.
- Endodontics and Periodontics may be moved to Basic Benefits and covered immediately. Deductibles and co-insurance apply.
- Annual Maximum Options - $1,000, $1,500, or $2,000 - You Choose
- Orthodontics may be added as a benefit - You Choose (Lifetime Maximum Options - $1,000, $1,500, or $2,000)
- Rates are guaranteed for 12 months from the effective date.
- Annual Open Enrollment Period
- Qualifies for Section 125 (Cafeteria Plan) Deductions
- Maximum of 3 Deductibles per Family
- Dependent children covered up to age 25.
- Takeover Provisions Apply
- Large Groups or Variations of Plan Benefits Require a Quote from Home Office
