• Some preventive and diagnostic services are provided at no charge
• Over 180 procedures are covered by co-payments.
• Must Select Dentist from DINA Network of Dentists. Network Includes Dentists Across the State of Louisiana
• Qualifies for Section 125 (Cafeteria Plan) Deductions
• Operating in Louisiana Since 1978

To apply for Dina Dental insurance products you can:

• Visit one of our five convenient locations and sit with an agent to apply. Please call (504) 441-RATE (7283) to make an appointment, or
• Complete our online quote request form and have someone call you back to take your application, or
• Call us directly at (504) 441-RATE (7283) and speak to one of our Dental Insurance Specialists, or
• Click on the link below to go directly online to the application for the product you want, print it and fill out as much as you can. Then you can fax it to (504) 454-3988, or mail it to our Life & Health Department at 3008 David Drive, Metairie LA 7003, or bring it to one of our five convenient locations.
